We are steadfast in our commitment to delivering the highest quality of service to all our clients, particularly those referred to us for Workers' Compensation matters. To ensure an organized and efficient process, we kindly request your adherence to the following guidelines when referring your clients for psychiatric services.

  1. Please review MindHope Welcome Letter, which includes Appendix A, to view our fee schedule and other important information.

  2. BEFORE completing the Intake form (see attachment below), please notify us via email if there are any associated medical records with the referral. Provide the total number of pages. Note that double-sided copies are considered two pages.  Please be aware that cases with extensive medical records may be subject to limitations to ensure an expeditious process. This information is crucial for billing purposes. 

  3. The Intake form can be completed by the person requesting psychiatric services ( please note that any omissions or incomplete details may result in delays). The Intake form must be returned within 48 hours, along with submitting the First Report of Injury (FROI). Timely submission is crucial for an efficient referral process. However, once the intake form is completed and returned, we will prepare  MindHope fee agreement along with an invoice. If the referral is for Psychiatric Evaluation and Treatment,  please note that contracts and Invoices will be forwarded directly to the Adjuster for review and signature, as the person who holds the authority to approve or deny the treatment

  4. Prepayment is mandatory fourteen (14) business days before the scheduled appointment. Failure to receive prepayment by the specified due date will result in appointment cancellation. Note that the Invoice will include the client's appointment date and time.

  5. New Patient Forms: Advise your client to visit our website at www.mindhopehealth.com to diligently complete the new patient forms. Kindly request that they bring the completed forms to their scheduled appointment.

  6. Client Arrival: Inform your client to arrive 15-20 minutes early for their appointment.
